KUALA LUMPUR: A multi-vehicle collision involving a tipper lorry and 15 other vehicles along the Bandar Serenia toll plaza slip road towards Cyberjaya was clearly captured on video.

The incident, which occurred around 3.30pm on April 12, saw a tipper lorry descending the slip road before crashing into vehicles lined up on both sides of the road waiting to exit towards Cyberjaya.

Sepang district deputy police chief superintendent Shan Gopal Krishnan said in a statement today that a preliminary investigation showed the driver lost control of the lorry after experiencing a brake failure.

"Initial investigations revealed that the lorry, which was coming from Nilai, suffered a technical issue believed to be brake failure.
